چکیده
TOPIC: Critical Care TYPE: Medical Student/Resident Case Reports INTRODUCTION: The term "buffalo chest" is used to define a single pleural space with no anatomic separation of the two hemithoraces, seen in an American buffalo or bison. There have been less than ten cases reported where spontaneous bilateral pneumothorax was suspected due interpleural communication. connection associated major invasive thoracic procedures such as esophagectomy, cardiothoracic surgery, pacemaker and central venous catheter placements. We present rare case simultaneous chylothorax fashioned by CASE PRESENTATION: A 45-year-old male chronic smoker past medical history esophagitis presented one week cough reflux. He otherwise denied fevers, odynophagia, chest pain, weight loss early satiety. hypoxic on admission requiring 2 L oxygen supplementation. Physical exam revealed decreased breath sounds left lower lung base. Chest radiograph showed minimal atelectasis at CT large sliding hiatal hernia distention esophagus centrilobular nodules right middle lobe lobes. presumptive diagnosis aspiration pneumonia likely secondary hernia. Thus, he underwent laparoscopic para-esophageal repair EGD. Two days later, developed acute respiratory failure intubated. concerning for new effusions. Within hours, ultrasound guided thoracentesis draining four liters opaque, pink, non-purulent fluid (figure 1). Left drained only five mL. Pleural pH 7.48, elevated triglycerides (600 mg/dL) low cholesterol (50 indicating chylothorax. had tubes placed bowel rest. This explained resolution right-sided thoracentesis. Patient extubated thereafter discharged. DISCUSSION: report documents completely resolved sided phenomenon formed during suggesting chest. Spilt Milk: An Unusual Cause of Bilateral Chylothorax.
We report a case of bilateral chylothorax without evidence of chylous fistula in a 62-year-old man following total laryngectomy and bilateral selective neck dissection for laryngeal cancer. Chylous fistulae, a well-known complication of neck dissection, occurs following 1% to 2% of these surgeries. On rare occasions, the chyle leak may communicate with the pleural space, resulting in chylothora...
متن کاملIatrogenic “buffalo chest” bilateral pneumothoraces following unilateral transbronchial lung biopsies in a bilateral lung transplant recipient
We present a 54 year old male patient who had a bilateral lung transplant sixteen years ago for Alpha-1 Antitrypsin Deficiency-related emphysema. He was referred for flexible bronchoscopy with transbronchial biopsies to evaluate new mild exertional dyspnea and worsening of his FEV1. Eight transbronchial biopsies were done from the right middle lobe and the right lower lobe. Post procedure he de...

It is characterized by a high concentration of neutral fat and fatty acids in the pleural fluid. Bilateral chylothorax is even more...
متن کاملTraumatic chylothorax following a closed chest injury.
Two days later he felt discomfort in the chest and a skiagram showed re-accumulation of fluid in the right pleural cavity, and obliteration of the left costo-phrenic angle. Bilateral pleural drainage tubes were inserted and 3300 ml. of thin, whitish fluid on the right side, and 800 ml. of blood stained fluid on the left side were obtained on the first day. The left sided tube drained minimal am...
